What this website collects

Appointment requests. If you use our request form, we collect your name, phone number, email address (optional), whether you’re a new or existing patient, the days and time of day you prefer, and an optional short note. We use this only to contact you about scheduling. The form is not for health information — please don’t include symptoms, medications, insurance numbers or other health details.

Page counts. We count visits to pages with a privacy-friendly analytics tool that runs on our own website, doesn’t use cookies, and doesn’t identify you. Our appointment request, contact, new-patient and patient-forms pages don’t use analytics at all.

Server logs. Like most websites, our web server keeps short-term technical logs (such as IP address, browser type and the page requested) for security and troubleshooting.

What we don’t do

• We don’t use advertising pixels or tracking cookies, and we don’t sell or share your information for advertising.
• We don’t ask for your date of birth, Social Security number, insurance member ID or payment card on this website.

Where requests go and how long we keep them

Requests are sent to our office by a secured email service (to be confirmed). A copy is kept in the website’s private admin area for up to 30 days so our team can follow up, then deleted automatically.

Maps and outside services

The Google map on our contact and home pages loads only if you choose “Show map”; until then, no information is sent to Google. Our online patient forms are provided by our patient-communication service, and any online payment or financing links take you to that company’s website. Those services have their own privacy terms.
