Fill out your forms online
Save time at check-in with our secure online patient forms. (to be confirmed)
Come in for your first visit
Bring your photo ID and insurance card. We’ll get to know you, look at your teeth and gums, and talk through a plan together.
What to bring
- A photo ID
- Your dental insurance card, if you have one
- A list of medicines you take (we’ll go over it privately at your visit)
- Your completed patient forms, if you printed them
- Glasses, if you need them to read forms
Your first visit
Plan on (to be confirmed). We’ll review your health history, check your teeth and gums, take X-rays if your dentist recommends them, and talk through anything we find — what needs attention now and what can wait. If a cleaning is appropriate, we may be able to do it at the same visit. (to be confirmed)
Feeling nervous?
Many people feel anxious about the dentist. Tell us when you call or when you arrive. We’ll explain each step before we do it, and you can ask us to pause at any time. (to be confirmed)
Insurance and payment
We file your insurance claims for you. We accept Cash, check, Visa and Mastercard. See insurance & financing for plans and payment options.
If you need to cancel
If you can't keep an appointment, please give us at least 24 hours' notice so we can offer the time to another patient. A fee may be charged for missed appointments without notice.
